Whenever possible, Mercy Hospice works with patients and families to keep them at home in familiar surroundings at the end of life. Hospice team members come to the home to provide nursing care and support services. Delivery of durable medical equipment can be arranged to facilitate keeping patients at home.
Sometimes, however, it’s just not possible or safe . . .
At times, it is just not possible for patients to remain at home. In these cases, Mercy Hospice is available to assure a smooth transition from home to area nursing facilities and assisted living centers. We have developed strong relationships with staff members at these facilities, working with them to provide the same specialized end-of-life care available in the home.
