Referral Process

Programs and Services for Adult Women

Referrals for halfway house services/programming should be directed to the Adult Programs Client Services Coordinator, by phone (515-643-6500), mail or email the Adult Programs Client Services Coordinator. An intake interview will need to be scheduled (a telephone interview can be arranged if the applicant is incarcerated, in residential treatment or not within reasonable driving distance).

At the time of intake, the following information will be requested:

  • pertinent client demographics
  • relationship status
  • family composition
  • medical history
  • chemical dependency history
  • mental health history
  • domestic violence history
  • other services the client is receiving
  • releases to pertinent service providers

At the conclusion of the assessment interview the client will be informed about her eligibility for House of Mercy programming and provided an estimated time frame for admission. Clients may be asked to attend/participate in alternate programming while waiting for admission, such as attendance in an Outpatient Treatment program. Lack of follow-through with any such recommendation can have a negative effect on the individual’s admission eligibility for halfway house programming/services. 

Requests for outpatient services/programming can be made by calling (515) 643-6500. An intake interview will need to be scheduled. At the time of intake the following information will be requested:

  • pertinent client demographics
  • relationship status
  • family composition
  • medical history
  • chemical dependency history
  • mental health history
  • domestic violence history
  • other services the client is receiving
  • releases to pertinent service providers

At the conclusion of the interview, the client will be informed of her acceptance/denial for House of Mercy programming.

Programs and Services for Adolescent Women

Referring workers may contact Leanna Brady, adolescent program supervisor, by phone (515-643-6500), mail or email lbrady@mercydesmoines.org. Please provide basic information regarding the client to determine eligibility. An active referral must include a social history with information about the child and a family history. Educational, medical, psychiatric and psychological information is also helpful, if available. Qualifying admissions will be asked to provide Placement Agreement; 3055 CACT authorization; court order; title 19 and/or other insurance information/cards. Guardians are to be present at admission if releases have not been signed prior to admission.
