You deserve the highest quality care from your health care providers. In order to provide the care you expect and deserve, health care providers need the most recent and accurate information about your medications and your medical history. Without this communication, there is increased risk for medication errors and side effects.

  • Know how to use your medications safely.
  • Don’t be afraid to ask questions about your medications.
  • Knowledge is power! Educate yourself and inform your family of your medical information in case of emergencies.
  • Download a MedCard (PDF)* and write down your most current information. Carry it with you at all times. Disponible en espa├▒ol (PDF)*.
