Geriatric Assessment

As our loved ones age, we become more and more concerned about their health and safety. Eventually, they may need assistance to maintain their independence. Often it’s a relative who first recognizes this need. However, making this decision can be very difficult. Ask yourself:

 

  • Do you worry about your mother’s unsteadiness on her feet?
  • Are you afraid your father forgets to take his medications?
  • Do you question whether your family can care for a parent or spouse who has Alzheimer’s?
  • Have you ever wondered whether your grandfather would benefit from nursing home care?
  • Does a family member want to return home from a nursing facility, but you’re not sure she should?

If you answered “yes” to even one question, perhaps your relative could benefit from an evaluation by the geriatric staff at Mercy Home Health Services. Allow them to help you find the best solution for your family.

Our goal is to make it possible for individuals who can safely function to remain in their own homes as long as possible - and to offer alternatives when necessary.

We will assess several aspects of your loved one’s situation including:

  • Medical condition
  • Social needs
  • Functional abilities
  • Home environment

Even if your insurance doesn’t cover geriatric assessments, you can still contract for an assessment through our private-pay program. Private-pay services are designed to help our clients remain independent in their own homes and for people who have complex needs. Your case manager will:

  • Tailor a care plan for your loved one
  • Routinely stay in touch with your relative’s doctor
  • Keep you informed

In both geriatric-assessment and case-management situations, after a complete assessment, our experts will discuss with you and your family member their recommendations. And, if you’d like, they’ll help you follow through with your chosen plan.
