Registration occurs at the Mercy Center for Pain Medicine office located on the third floor of the West building at Mercy Medical Center. The West Entrance is located on 5th Street just north of Laurel Street. Valet parking is available or patients may park in the West Ramp or North Visitor Parking areas. We ask that patients arrive 30 minutes prior to the scheduled appointment time to complete the registration process.
It is important for us to know if you cannot keep your scheduled appointment to help facilitate access for other patients. We understand that emergencies do occur, but we do ask that you please cancel appointments as far ahead of appointment time as possible. It is our policy that if a patient has not arrived, or has cancelled 2 (two) appointments (barring emergencies), the patient will need to contact their referring physician for Pain Center re-admission procedure.
Before their first appointment, patients will receive a mailing that includes all the information about their appointment. Patients should complete the included forms and bring them, along with any other requested information, to the appointment.
These items include:
- A list of your medications / dosages
- Your x-rays and other films
- The included pain history form filled out prior to coming
- Any/all copies of medical records pertaining to past pain management
- A friend or family member to drive you home following the procedure
