Copy of Paper Medical Records
To request a copy of your medical records for health care received by Mercy Medical Center – Des Moines, please follow the steps below.
- Print the Authorization for Access, Use or Disclosure of Protected Health Information form in English (pdf).
- Fully complete the authorization request in detail, including the patient’s signature and date of signature, located at the bottom of the form. If the patient is unable to sign, please provide the appropriate legal documentation needed to prevent any delays in retrieving records.
