Application Process
To apply to the program, you must submit:
1. A completed application that can be downloaded below.
2. Three letters of reference. Please download and use the reference form below. Two letters of reference must be from professors, laboratory instructors, teaching assistants or college advisors. One letter may be from an employer. References from relatives are not acceptable.
3. Official transcripts from all universities and colleges attended. If any part of your education was obtained outside of the United States, you must have your transcript evaluated by a qualified agency for United States education equivalency. A list of such agencies may be obtained by contacting us at the email address shown.
4. A one-page typed essay describing why you chose clinical laboratory science as a career and your future plans as a clinical laboratory scientist.
5. A $25 application fee.

Application deadline is November 1 of each year for the class beginning the following August.
