Health Care:
CHI Medical Plan
Shared cost by you and Mercy Point-of-Service plan with out-of-network coverage. Preferred panel of providers; Mercy is the network hospital. Prescription coverage; vision coverage.
Dental Care – 2 options:
Delta Preferred Option
Delta Premier Plan
Paid by you Delta Dental providers
Long Term Disability Core plan paid by Mercy. High option plans shared by you and Mercy. Core Plan pays 50% of base monthly income. High option plans pays 60% or 66 2/3% of base monthly income. All plans pay following 90 days of disability. The policy goes into effect 90 days after employment.
Short Term Disability Paid by Mercy 60% of your base weekly earnings or a flat amount from $100 to $500 a week. The policy goes into effect after 90 days of employment.
Basic Term Life/Accidental Death Insurance Paid by Mercy 2x Annual Salary

Coverage paid by you. Coverage on spouse and children paid by you. Maximum benefit of 5 x annual salary or $750,000 on yourself. Spouse maximum of $200,000; child maximum of $20,000. The maximum takes into account any basic life provided by Mercy.
Reimbursement Accounts Elect pre-tax dollars to pay for out-of-pocket health care costs and dependent care.
Tax-Deferred Annuity Plans Pre-tax contributions to retirement account.
