Dental Care – CHI Dental Plan Preferred Option Paid by you MetLife Preferred Provider network. Benefit coverage for preventive, diagnostic, basic and major restorative services
Vision Care Paid by you Vision Service Plan. Covers eye exams and hardware.
Long Term Disability Paid by Mercy Pays 60% of base monthly income after 6 months of disability. The policy goes into effect 90 days after employment.
Short Term Disability Paid by Mercy Pays 80% of weekly earnings for the first 6 weeks, then 60% from week 7 through week 26. The policy goes into effect after 90 days of employment.
Basic Term Life/Accidental Death Insurance Paid by Mercy 2x annual salary
Supplemental/Voluntary Life for self, spouse and children
Paid by you Maximum benefit of 5 x annual salary or $750,000 on yourself. Spouse maximum of $200,000; child maximum of $20,000. The maximum takes into account any basic life provided by Mercy.
Reimbursement Accounts Elect pre-tax dollars to pay for out-of-pocket health care costs and dependent care.
